If you are a new client, please fill out the Health Information form as well as the Screening Questionnaire form prior to your session.  These are general intake forms that help me gain a better understanding of your needs as well as if I need to take special precaution.  I reserve the right to refuse to work with anyone who is unwilling to disclose general health-related information provided on your intake forms. These forms are used to ensure that you are given proper care therefore are required to be filled out prior to your visit. Your information is kept private and confidential.
